Please use the form below to submit your immunization information and documentation. The deadline to submit this information is:

  • May 20, 2022 at 11:59pm CT for Summer Session I
  • June 3, 2022 at 11:59pm CT for Summer Sessions II and III
  • July 8, 2022 at 11:59pm CT for Summer Session IV

If you have any questions, or you are requesting a medical or religious exemption, please contact our office at summersession@wustl.edu.

Failure to provide proof of immunization or documentation for exemption by the deadline will result in cancellation of your enrollment.

COVID-19 Immunization Requirements

All students must be fully vaccinated against COVID-19 and must provide proof of immunization for COVID-19. We are following  CDC or WHO guidance and accept vaccination with either an FDA or WHO EUA or fully approved vaccine. As of June 1, 2021. the following vaccines currently meet this requirement:

  • Moderna-2 doses
  • Pfizer-2 doses
  • Johnson & Johnson/Janssen-1 dose
  • AstraZeneca (AZD1222) (ChAdOx1-S)-2 doses
    • SKBio /Republic of Korea)
    • Serum Institute of India (COVIDSHIELD)
    • AB/Sweden (VAXZEVRIA)
  • Vero Cell (inactivated) made by Sinopharm,  Beijing (BIBP)-2 doses
  • Vero Cell (inactivated) made by Sinovac Life Sciences Co, Ltd.-2 doses
  • Covaxin made by Bharat Biotech-2 doses

Washington University does not currently require boosters, but they are strongly encouraged.

Immunization Record Uploads

Evidence of required immunizations in the form of official medical documentation that includes your name and date of birth must be uploaded. Records must be uploaded in English (originally) or translated by a licensed professional translation service. JR Translation Services is one option for medical translation, but students are welcome to use any licensed service. The name and manufacturer of the vaccine must be included in the translation.
